Care Together: Community Programs for Older Adults Navigating Dementia or Frailty and their Care Partners This free program is for older adults living in their own home who are experiencing dementia or frailty and their care partners. Each session offers a welcoming and supportive environment where older adults and their care partners can take part as a pair and enjoy gentle fitness activities, followed by time to relax and connect over light snacks and conversation. Creative experiences such as painting, collage, or arts and crafts provide opportunities for shared enjoyment, self-expression, and meaningful engagement. Together, participants can build connection, support well-being, and enjoy time with others in the community. Care partners also have the opportunity to connect with peers, feel supported, and experience a lightened mental load in a friendly and inclusive space. This program has been generously sponsored by the North Simcoe Muskoka Specialized Geriatric Services Program under the leadership of Waypoint Centre for Mental Health Care.
